Eligibility

Any individual who has a disability or multiple disabilities that result in the following may be eligible for CityLift paratransit service: -Inability to get on an accessible fixed-route bus -Inability to disembark from an accessible fixed-route bus -Inability to get to or from, or wait at a fixed-route bus stop

Required documents

Must complete application and include a diagnosis of disability from physician on physician's prescription pad paper.

Peoria Area & East Peoria Routes Monday-Friday: 5:00 a.m. to 12:45 a.m. Saturday: 7:30 a.m. to 10:30 p.m. Sunday: 7:30 a.m. to 7:00 p.m. Pekin Routes Monday-Friday: 6:45 a.m. to 6:30 p.m. No Saturday or Sunday Service No service in any area on the following holidays: -New Years Day -Easter Sunday -Memorial Day -July 4th -Labor Day -Thanksgiving Day -Christmas Day

Application process

Please call for an application; visit the CityLink Transit Center (407 SW Adams) or the CityLink Administration office (2105 NE Jefferson Ave); or visit the CityLink website at www.ridecitylink.org. If in need of assistance to fill out the CityLift application, please call the CityLink Special Services Department at 309-679-8183 or call Advocates for Access at 309-682-3500. With each number, the individual should let the receptionist know that he or she is in need of assistance with the CityLift application. When the CityLift application is submitted to CityLink's Special Services office, a determination of eligibility will concluded within 21 days of submission of a properly completed application.

Fees

$2.00 per one-way trip or $4.00 per round trip

Service area

Peoria, Peoria Heights, and West Peoria. Limited service is also available in East Peoria and within a defined service area in Pekin.

Description

CityLift paratransit service is a “safety net” for only those individuals who do not have the functional capability to use the fixed-route system. Disability alone does not qualify a person to ride the paratransit service under the American’s with Disabilities Act.
